PLEASE READ THIS WAIVER & RELEASE IN ITS ENTIRETY BEFORE SUBMITTING THIS FORM.
In consideration of my/our child(ren) being allowed to participate in any way with the Legacy X Factor Athletics Non-Profit Organization, it's related events and/or activities, I/We acknowledge, appreciate and agree that:

* The risk of injury from the activities involved in this program is significant, including the potential of permanent disabilities, and in very rare instances even death, and while rules, equipment, and personal discipline may reduce risk, the likelihood of serious injury does exist and.

I knowingly and freely assume all risks, both known and unknown even if arising from the neglect of others, and assume full responsibility for my participation, and I willingly agree to comply with the stated and customary terms and conditions for participation in this youth sports activity. If however, I observe an unusual and significant hazard in my presence or participation, I will bring such to the attention of the nearest official immediately, and

I, for, myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the Legacy X Factor organization, its officers, agents, and/or volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and lessors of premises used in team activities, with respect to any and all injury, disability, or loss or damage of personal to person or property, whereas arising from the negligence or the release of otherwise, to the fullest extent by law.

IF A CHILD LEAVES A PRACTICE OR GAME FOR ANY MEDICAL REASON, HE OR SHE CANNOT RETURN WITHOUT MEDICAL CLEARANCE FROM A PHYSICIAN.

At any time may request medical clearance or a physical to be able to participate or play at his/her discretion.

PHOTO RELEASE: I also hereby consent, without further consideration or compensation, to use (full or partial) of all photos, audios or videos made of my child or family members during Legacy X Factor activities, for documentation, promotional materials, or web content. Further, I release Legacy X Factor and its volunteers from and liability which may arise from the use of such media.

I have read this release of liability and assumption of risk agreement. I fully understand its terms and I understand that I have up substantial rights by this agreement and agree to it freely and voluntarily without any inducement.
